Our Company

Seasons of Change LLC entered its 5th year of practice on February 1st, 2017 and continues to provide Aging Life Care services from a *holistic approach. We do not see individuals as a “set of diagnoses” but rather as unique human beings with many aspects impacting them that must be considered. Our services include: Transitional Care Management offered in our BRIDGE Program, Long Term Care Management, Quality of Life Services and a developing Product Line of tools designed to help you in coordinating the care and management of your loved one. Each of these services is designed to respect the dignity of and empower our clients to achieve their optimal quality of life.

Seasons of Change was founded by Teresa Wheeler who has a 21 year career in nursing serving the senior population. Concurrently she provided care giving to several family members. Teresa and her team of professionals are passionately dedicated to Empowering seniors and their families by providing them with the education, resources and strategies necessary to navigate the health care system, address the changing needs in the senior’s life and enable them to move as fluidly as possible between the seasons of their life. We believe that every senior deserves to enjoy the highest quality of life possible.  To that end we will evaluate our clients and their situations and provide them with a plan individualized to meet their particular needs.

Our Team



Teresa Wheeler

Teresa Wheeler is a nurse licensed in the state of Ohio and an Advanced Professional member of the Aging Life Care Association.  Teresa brings her 20+ years nursing background working with the senior population to her practice and approaches her clients holistically, respecting and addressing the many facets that must be considered when working with clients and their families. 

In addition, Teresa holds a BA in Human Services from Baldwin-Wallace University, is a Certified Patient Advocate having completed her coursework at Cleveland State University, she holds a Care Manager Certification through the National Academy of Certified Care Managers, and is currently working on her MA in Gerontology through the University of Nebraska at Omaha.  

Teresa, in her fifth year of practice along with her care team, work with seniors and their families by advocating for them and guiding them through the aging process, striving to provide them the tools and resources necessary to achieve their highest quality of life.

Denise Kennedy

Denise Kennedy serves as an Aging Life Care Manager with Seasons of Change Consulting.  She joins our team with over 10 years of experience as a Registered Nurse in a hospital system; with the last four years serving as the community liaison for home health.  She brings experience in setting up post-acute home care for hospitalized, and skilled nursing patients; while also providing links to community service organizations, DME, and private duty care.  She graduated with a Bachelor of Arts from BGSU in 1992; worked for 10 years in a business environment; then returned to school to acquire her RN in 2007.  Denise looks forward to helping seniors and their families find the appropriate quality care and assistance that allows them to live a comfortable, fulfilling life at home.

Becky Spilak Salamon

Becky Spilak Salamon serves as an Aging Life Care Manager with Seasons of Change Consulting.  She joins our team with over 14 years of experience as an Licensed Independent Social Worker primarily in hospital systems in the Cleveland area; with the last eight years serving in Geriatric Programming including Geriatric Assessment, Home Health and Hospice as well as a grant-funded community program which reached out to seniors in their homes.  She brings with her strong clinical skills in working with clients across the life span and a collaborative, team-oriented philosophy. 

She graduated with a Bachelor of Arts in Communications from Oakland University in Rochester, MI in 1992 and a Master’s Degree in Social Work from Wayne State University in Detroit Michigan in 1997.  Additionally, Becky has worked as a Case Manager in a Long Term Acute Care, LTACH, setting and also has corporate experience in Marketing Project Coordination and in the development of educational, marketing and training collateral as well as event and speaker coordination.  Becky was  awarded the President’s Award for Quality Performance from Southwest General Hospital in 2010 for initiating, developing, and managing an employee fundraising event resulting in raising $20,000 for the hospital foundation. 

Becky is passionate about helping seniors and their families find the appropriate programming and assistance that will allow them to live a comfortable, fulfilling life at home.
