Seasons of Change LLC entered its 5th year of practice on February 1st, 2017 and continues to provide Aging Life Care services from a *holistic approach in Ashland, Cuyahoga, Erie, Lorain, Medina, Portage and Wayne Counties.. We do not see individuals as a “set of diagnoses” but rather as unique human beings with many aspects impacting them that must be considered. Our services include: Transitional Care Management offered in our BRIDGE Program, Long Term Care Management, Quality of Life Services and a developing Product Line of tools designed to help you in coordinating the care and management of your loved one. Each of these services is designed to respect the dignity of and empower our clients to achieve their optimal quality of life.
Seasons of Change was founded by Teresa Wheeler who has a 21 year career in nursing serving the senior population. Concurrently she provided care giving to several family members. Teresa and her team of professionals are passionately dedicated to Empowering seniors and their families by providing them with the education, resources and strategies necessary to navigate the health care system, address the changing needs in the senior’s life and enable them to move as fluidly as possible between the seasons of their life. We believe that every senior deserves to enjoy the highest quality of life possible. To that end we will evaluate our clients and their situations and provide them with a plan individualized to meet their particular needs.

Teresa Wheeler is a nurse licensed in the state of Ohio and an Advanced Professional member of the Aging Life Care Association. Teresa brings her 20+ years nursing background working with the senior population to her practice and approaches her clients holistically, respecting and addressing the many facets that must be considered when working with clients and their families.
In addition, Teresa holds a BA in Human Services from Baldwin-Wallace University, is a Certified Patient Advocate having completed her coursework at Cleveland State University, she holds a Care Manager Certification through the National Academy of Certified Care Managers, and is currently working on her MA in Gerontology through the University of Nebraska at Omaha.
Teresa, in her fifth year of practice along with her care team, work with seniors and their families by advocating for them and guiding them through the aging process, striving to provide them the tools and resources necessary to achieve their highest quality of life.
